An immediate denture may be the best solution when complete extractions of your remaining teeth become unavoidable. If all of the remaining teeth in either the upper jaw or the lower jaw, or both jaws, cannot be saved, it would be necessary to extract them all and get dentures instead.

If all the teeth were extracted and the gums and bone would be allowed to heal up, it would be a few months before the new teeth could be placed. During that time, you would have to walk about without any teeth in your mouth. That would be an extremely embarrassing and unthinkable nightmare. An immediate denture will allow you to avoid the embarrassment of living without teeth, even for a single day, even for a few hours.  We will begin the preparation for this procedure before your teeth are extracted.

  1. The first step is taking impressions of your existing teeth and gums to create accurate duplicates for placement after tooth removal. We will also pick a shade for the new teeth, and take some more measurements.

  2. In a few days, you will return, with your own loose teeth still in your mouth. Dr. Paghdiwala will give you local anesthesia to numb your mouth, extract all your teeth painlessly and place your new denture immediately in your mouth. He will also place a soft reline inside the new denture to provide a cushion effect on the healing gums. You will be given instructions on proper care of the new denture and you will be on your way home. Antibiotics will be prescribed and you will be asked to come in the next day for a quick  follow-up visit.

  3. If you are a bit apprehensive about the whole process, especially about having multiple teeth taken out in one visit, and that too without being put to sleep, Dr. Paghdiwala can consult your medical doctor to have them prescribe some sedatives to help with the anxiety. You will be very impressed with how easy the whole process of replacing loose and failing teeth is. 

  4. As an alternate, you can choose to have the back teeth (that are not critically important from the esthetics point of view) taken out gradually, and only have the front teeth remaining in the mouth for extraction on the day you will be getting the new teeth.

  5. Dr. Paghdiwala also recommends that dental implants be placed to support the new teeth. Very often, dental implants can be placed right at the time of extraction of the loose teeth, before placing the new immediate denture. 

  6. And even if you choose not to get implants on the day of the extractions, we always recommend that bone grafting material be placed immediately into the sockets of the extracted teeth.  This is because, if nothing is done after the teeth extractions and the bony sockets are left alone to heal naturally, there is almost 40% loss of bone following  extraction of teeth. The bone grafting will preserve the bone and keep the option open for implants should you ever choose to get them sometime in the future.

After Extraction

Great care should be taken to follow all postoperative instructions. Your gums and bone will recede after extraction, and regular check-ups and maintenance will monitor healing and reduce problems. Temporary linings or tissue conditioners will be required to create an optimal fit and may need to be adjusted or replaced a number of times during the healing period. A permanent denture reline will be required to ensure a comfortable and exact fit after you have completed the healing process.
